The Michels & Gauquie Dental Wellness Plan is an in-office discount plan designed to assist patients without dental insurance who would like to invest in their smile. We understand the difficulties and complications that can arise when working with insurance companies. Whether it’s high out-of-pocket costs, low maximums, or lack of procedure coverage, we understand that insurance doesn’t always benefit our patients as much as it should. This is why we developed the Michels & Gauquie Wellness Plan, to be used exclusively at Michels & Gauquie. The Michels & Gauquie Wellness Plan is a discount plan designed to help you keep up with your preventative visits and get any needed treatment done in an affordable manner. There is no maximum, no deductible, no waiting period, and no claims or denials.

Qualified additions to the primary subscriber include the applicant’s legal spouse and any non-married children under the age of 22.
Exclusions and Limitations
This plan is not insurance. This plan is not refundable under any circumstance. Appointments are subject to availability, and lack of availability does not constitute a refund or additional discount. If a service covered at 100% is not used by the expiration date of the plan, the patient forfeits that service and it does not rollover. This plan cannot be combined with any other discount or insurance. This plan runs for 365 days, including the day the premium is paid. Office fees are subject to change at the office’s discretion. Any procedure in progress or performed before the member’s eligibility date is excluded. This plan can only be used at Michels & Gauquie, and it is not applicable if the patient is referred to a specialist’s office. The doctor will work with the patient to decide the best course of treatment. If third party financing is used, the General Care Courtesy will be reduced to 10%, and the Specialty Care Courtesy will be reduced to 5%.
